Pixie Preschool - Handbook & Forms

In accordance with school policy, Pixie Preschool offers the following required forms for parent download and use.

Registration & Emergency Contact Form – Please use this form to register your child in the Pixie II Preschool classes.

Health & Immunizations Form – Required by the State of New Hampshire, this form must be completed by your child's physician prior to starting at Pixie II. Please note that all immunizations must be up to date prior to your child's first day of class. This form may be returned to us with your child or via fax.

Student Profile Form – This form provides staff and teachers with information vital to creating a nurturing environment for your child. Please download this form and return it completed and signed to your child’s teacher.

Summer Program 2009 Registration Form – Please use this form to register your child for the Pixie Summer Programs and extra curricular activities for 2009.

Pixie Preschool II Handbook – The handbook is provided to explain school policies and procedures to the parents of children in our Preschool and Pre-Kindergarten programs. Information regarding Before & After School, Moose Hill Enrichment and Extended Care programs is also provided. Please note that we require parents to sign and return the Handbook Acknowledgement form upon receipt of this handbook.
